TERMS AND CONDITIONS NEW COMMERCIAL ACCOUNT AND CREDIT APPLICATION

In addition to any terms and conditions associated with the Authorized Dealer Agreement, if signed by the parties, the above-named applicant ("Applicant") agrees to the following:

Applicant acknowledges that the contents contained in this application are true and accurate, and relied upon by FORCE3 for purposes of entering a commercial resale relationship with the Applicant.

Credit Check Authorization. By agreeing below, Applicant authorizes FORCE3, directly or through a third-party provider, to obtain a credit report applicable to the Applicant. This authorization is valid for the purposes of verifying information given pursuant to any lawful purposes, including the purposes covered under the Fair Credit Reporting Act ("FCRA"). Further, the Applicant authorizes all corporations, credit agencies, law enforcement agencies, city, state, county, and federal courts and agencies, and any other persons to release all information they may have about the Applicant. This authorization shall be valid in original or copy form.

Applicant agrees to sell FORCE3 product on their own website and physical retail locations only.
